Added anonymous record in database table Django Mysql Amazon AWS Hacked

My Django website is hacked, and a new record is created in my database table, I checked the django request_request, and there is no entry, checked the logs no entries about this new record.

Can you please help me how this can happen and how to prevent this. Is this hacking on the Amazon Server or the Django level.
